A/C then hit the top of a small 2-story building beside Hwy QL 20, just north of the FSB. A/C hit the top of the building then hit the ground at a high rate of descent where it exploded on impact. Witness then observed that one of the top lines from a group of high tension lines that spanned the road was broken.

A/C was on a recon mission out of Bu Prang. Team consisted of 2 LOH and 2 Cobras. At approx 1600 hours while 5 miles E of Bu Prang, 3 foxholes along QL14 were spotted. Approx 5 miles E of the location where we took fire, the other LOH signaled for crew to put the ship down. The ship was put down in an open field near QL14 (YU793658). P and OB got in the other ship and during take off, the ship received heavy machine gun fire. The LOH tore apart, fell to the ground and burned.

Pilot tried to avoid a tall tree by climbing and banking hard right, however corrective action was too late and A/C impacted with tree. The left skid and tailboom broke from A/C and started to spin as it fell. A/C hit more trees and crashed inverted.

Two scouts were operating with an USAF spotter plane in a hot area north of Nui Ba Den, near the Cambodian border. The team engaged the NVA in order to determine the enemy's strength and exact location while the FAC prepared to call in an air strike. Both ships came under heavy fire from multiple .51 & .30 cal machine guns, and A/C was shot down, crashed through some trees and immediately caught fire.

A/C was taking off on mission. At altitude of about 50' heading north and about 20 yards from LST, pilot made a right turn and headed east. It appeared that he was going to circle around the ship to gain airspeed. A/C gradually began to lose altitude to a point where A/C was not more than 20' above the water. As A/C leveled off heading east into wind, the skids skimmed the water, A/C tilted forward, crashed into the river and sank rapidly.

A/C was on a test flight due to a high freq vibration. During hover checks, A/C began turning approx 90 deg to right, moved backward and tail pitched down contacting the ground. Lower stabilizer was bent allowing T/R blades to hit ground, T/R drive shaft twisted in two, acft pitched forward causing M/R blades to strike PSP, impacted the PSP on skids, made 10-12 violent right turns, rolled on its right side and spun 3-4 more times before stopping.

A/C was dispatched to the area near LZ Grant. The pair of scouts landed at LZ Grant before heading out to the area where the first team came under fire. As they came into a clearing, they came under heavy fire from an AA emplacement and was immediately hit. Fire was seen to be coming from the engine compartment area as the ship nosed down, hit hard, broke up and began to burn. No one was seen exiting the helicopter.

At an altitude of about 90' and 60 KIAS, the A/C's engine stopped. Although an autorotation was attempted, A/C had neither altitude nor airspeed. The A/C was moving at 30-50 knots when it hit the ground, impacting on its left side in a nose-low attitude and continuing on for about 50 meters before coming to rest.

A/C was assigned to transport a prisoner from a Navy river boat on the Song Vam Co Dong River. After departing the boat, the A/C followed the river toward its destination. During the trip, a skid contacted the water and and the A/C crashed into the river. Elrod drowned while swimming to shore.
