After a second explosion, the A/C descended vertically, the M/R separating from the A/C. The A/C crashed in an unsecured area and burned. One of the ARVN troops popped off an M-79 round accidentally that caused the crash.

Upon arrival at the Cu Chi Airfield the flight received permission to land and discharge their cargo; however, on base leg of the approach the lead Little Bear sustained T/R drive shaft failure, apparently caused by an empty plastic water jug falling from the A/C and striking the T/R, resulting in loss of control, causing the tailboom to separate and crash.

On takeoff, A/C had an engine failure, hit an ant hill, broke in two, ruptured the fuel cell and caught fire just outside the airfield perimeter. *Gunner died 2 days later from burn injuries.

The Rattlers were on an afternoon scramble with the LZ being a laterite area. The second V of 5 was taking off when the lead ship put it back on the ground. A/C was in the #3 slot (right side) behind lead and in IFR conditions (ground dust) did not know that lead stopped. A/C flew into the rotor system of lead ship.

According to AC, the A/C never received hostile fire before impact. The RMI had pre-cessed during flight after being caged on preflight and they had just received a H&I fire mission to the west of An Khe from Hard Anvil, directed pilot to fly west. In reality they flew more S-SW and flew into the mountain SW of An Khe.
